HeartLinQ: Advancing Remote Care for Heart Failure patients

Project Overview

Heart Linq is an innovative project led by AIKON Health focused on improving the care for people living with heart failure (HF). The project brings together research institutions and innovative SMEs to develop a non-invasive wearable solution for continuous, remote monitoring of Heart Failure patients. Heart failure places a significant burden on patients, healthcare professionals, and healthcare systems, often resulting in frequent hospital admissions and reduced quality of life. HeartLinQ addresses this challenge by enabling continuous  physiological monitoring outside the hospital, supporting earlier detection of clinical deterioration and more timely clinical interventions. By translating physiological data into actionable insights, the project aims to support proactive, personalized care while reducing avoidable hospitalizations. HeartLinQ is funded under the OPZuid 2021-2027 programme, co-financed by the European Union, and reflects the strength of European collaboration in advancing scalable digital health innovations.

Project Objectives

• Develop a non-invasive wearable prototype capable of continuously monitoring heart failure relevant physiological parameters, including:
  • Thoracic impedance
  • Heart rate
  • Respiratory rate
• Develop and refine algorithms that translate physiological data into real-time, clinically meaningful insights for healthcare professionals.
• Validate the technology through analytical and clinical studies, ensuring accuracy, usability, and patient comfort.
• Demonstrate the feasibility of continuous remote monitoring in real-world care settings.

Expected Results and Deliverables

By the end of the project  HeartLinQ aims to deliver:

• A functional wearable prototype ready for analytical and clinical validation.
• Analytical validation study reports involving healthy volunteers.
• A clinical validation study including approximately 40-50 heart failure patients.
• Validated algorithms for real-time physiological monitoring and early detection of deterioration.

A comprehensive final project report summarizing outcomes, insights, and learnings.

Impact

HeartLinQ is expected to contribute meaningfully to the future of digital health and remote patient monitoring. By enabling continuous monitoring and earlier intervention, the project aims to improve patient outcomes and quality of life while helping healthcare systems transition toward more proactive, data-driven, and patient-centered care models. The results are designed to be scalable and applicable across real-world clinical settings.
